Your First Dental Visit
Like every other visit you’ll have here at Smile Rejuvenations, your first visit will revolve around your unique needs. You’ll be introduced to our team, be shown around the dental office, and then you’ll be given a thorough exam and have some X-rays taken. You’ll then meet with your dentist in a private consultation room where they’ll go over their findings with you, discuss your options, and answer your questions before putting together a treatment plan based on your goals.
New Patient Forms
We never want to waste your time, so to that end, you can fill out our patient forms before you’re sitting in our dental office. Just fill out the form prior to your appointment, and this will enable our team to check you in almost immediately.
Dental Insurance Welcome
Smile Rejuvenations is able to work with dental insurance plans from almost any provider. As a courtesy on the day of service, we will electronically submit your dental insurance claim on your behalf. Payment is due for insurance and non-insurance patients at the time of service and many dental insurance providers can reimburse patients for services. If you have questions about your coverage, we can answer those as well, so be sure to give us a call!
